MONA VALE GOLF CLUB - REGISTRATION FORM & TAX INVOICE Golf Avenue, Mona Vale, Thursday 2 February 2012 from 6 to 9pm Registration Closing Date: Form + payment must be received by Friday 9th December 2011 to secure a place (unless filled earlier).
Note: 1 Exhibitors are supplied a trestle table, a white tablecloth & two chairs. If you have additional essential requirements, such as extra chairs, music performance space, access to power, etc, please indicate in the Special Request box below. 2 Power and wall space cannot be supplied to all exhibitors. The expo floorplan is based on information supplied in the Special Request Box on the expo registration form. If you do not specify that power is essential, then you should presume it will not be available. In any case no guarantee is given that special needs can be met as each venue is different. 3 If your online banking forms do not allow for a business name to be left as a reference, then please enter your payment number in the Special Request box so your payment can be matched to your registration. 4 Registration is confirmed only when payment is received. 5 When you submit this form, a copy is sent to your email address. This is your Tax Invoice and a receipt will not be issued. 6 A full list of exhibitors at this expo will be uploaded to our site about a week before the expo, together with a floorplan. 7 If the form does not transmit correctly, please print off a hard copy and fax to 9400 2902. 8 PLEASE USE THE SPECIAL REQUEST BOX TO ADVISE NBWE OF ANYTHING THAT NEEDS SPECIAL ATTENTION
